Please find below a selection of forms that are required for this service. It is important that you understand the context of the advanced services before using these forms.

Medicines Use Reviews - Important reminder
From 1 October 2008 only the New version 2 MUR form can be used.

Please visit the Advanced Services section of the website for more information on the Medicines Use Review Service. Copies of the forms in use in Wales can be found on the Community Pharmacy Wales website (www.cpwales.org.uk)
