MUR data requirements

Advanced ServicesThe national NHS MUR form must be used to record the details of all MURs provided by pharmacy contractors. It is also used to communicate relevant information to the patient's GP. Pharmacy contractors must also be able to provide a summary of their MURs conducted on a quarterly basis to their PCT.

Most PMR systems contain MUR modules that can be used to record the details of MURs conducted and print off the completed NHS MUR form.


How do I use the NHS MUR form?

The form is used to record the details of the MUR. A copy of the completed form should always be provided to the patient. 

Pharmacy contractors are required by the Directions to communicate with the patient's GP following an MUR:

a) When there are items within the Action Plan that need to be considered by the GP/Practice pharmacy contractors are required to send the Overview page of the form to the patient's GP within 7 days of conducting the MUR. The pharmacist should make a professional judgement as to whether it is necessary to provide the GP/Practice with the Consultation record page of the form. If the Consultation record page is not sent to the patient's GP at the time of sending the Overview page, then the GP will be able to request a copy of it, from the pharmacy should they require this.

b) When there are no items within the Action Plan that need to be considered by the GP/Practice then a completed copy of the MUR form need not be sent to the GP. In this circumstance the pharmacy contractor should notify the patient's GP that an MUR has been undertaken, without any matters for consideration by the GP, within a month of the MUR being conducted. The notification should indicate that the GP can request copies of the completed MUR form(s) from the pharmacy. A template notification form can be found by clicking on the link below:

The Directions state that the pharmacy contractor should keep a copy of the completed MUR form for at least two years following the date on which the MUR was conducted. This record can be kept in an electronic format.


MUR data requirements

As part of the changes to the pharmacy contract in October 2011 it was recognised that in order to demonstrate the value of the MUR service it is essential that data is collected to show the benefits of the service to patients. This required a number of changes to MUR systems and paperwork to facilitate the data collection and NHS Employers and PSNC agreed a two-phased approach, with some changes being introduced in October 2011 and the remainder being introduced in April 2012. These changes are summarised in the following document:

Each pharmacy providing MURs must complete the nationally agreed reporting template (see link below) by collating the necessary data from pharmacy records for the MURs conducted in that quarter. Pharmacies must ensure that the completed data is available to be requested after the end of 10 working days from the last day of that quarter (last day of June, September, December and March). Completed templates must be provided to the PCT or successor organisation on request (which may be an ongoing request).

Collating MUR data for submission to PCTs

PSNC has developed an Excel spreadsheet which will allow pharmacy contractors to collate the necessary data from pharmacy records of MURs each quarter for submission to the PCT:

Alternatively a paper based approach can be taken to collating this data throughout the quarter, so that you can complete the spreadsheet for emailing to the PCT at the end of the quarter. The following form has been designed to allow this paper based collation of data:
