Local Arrangements
Local Registration Authorities (RA) are responsible for registering pharmacy staff for smartcards. PCTs have responsibility for establishing their own Registration Authority service – this may be within a primary care trust, as part of a shared local health community service or a wider shared service.
All Pharmacy staff registering for a Smartcard for the first time will have to go through the full registration process which includes identity checks that satisfy government standard ‘e-GIF Level 3’. This means providing at least three forms of ID (photo and non-photo), including proof of address and attending a face-to-face meeting with the PCT Registration Authority. Contact your local PCT Registration Authority or LPC to find out what arrangements are in place locally for this meeting, for example Registration Authority visits to pharmacies or pharmacy specific sessions at the PCT.

Blackburn with Darwen and East Lancashire PCTs
The majority of pharmacists in the area have now been registered and have collected their Smartcards. If you have not been registered, or have not yet collected your card, please contact the Registration Authority (details below) to make an appointment.
Smartcard certificates will expire after 2 years; therefore the earliest Pharmacist Smartcards in this area will begin to run out at the start of April 2008.
Certificates can be renewed by the Registration Authority up to 3 months prior to expiry and will avoid the user being denied access to the Electronic Prescription Service.

Buckingham EPS Registration Information
Milton Keynes
All pharmacies have received EPS01 forms and instructions from Janet Corbett, Chief Pharmacist. These should be returned to her completed, but unsigned, by all pharmacists and locums. Please make as many copies of the form as you need.
One of the Registration Authority officers will contact you to make an appointment to check documents and take photographs. They will arrange a time to return to issue your cards, and check with you that they work.
If you are a locum, and have not received a form, please make an appointment to visit the RA Manager at Milton Keynes Hospital. Your smart card will be issued at the time.

If you have not already obtained your smartcard, please contact the PCT urgently, particularly if you are a locum pharmacist who has not already registered with another PCT.
To obtain your smartcard, please contact Stuart Ward (System Access Administrator) on 01384 366993, or Jim Dempster (RA Manager) on 01384 366609 for an appointment between the hours of 9.30am and 4.00pm. You will need to visit the PCT offices in St John’s House, Union Street, Dudley.

Smartcard issuance information for pharmacists working or resident in the Greater Nottingham Area:
Times: Smartcard sessions for Pharmacists are held on the first Wednesday afternoon of each month. Appointments at these sessions are available in 15 minute slots from 2pm-5:30pm.
